Required Vaccinations for 2016/17

Dear Parents and Guardians,

It is not too early to start thinking about scheduling doctor’s appointment for your children for the upcoming 2016-2017 year.  We would like to remind you that all students new students and those entering Preschool, SEL, Grade 5 , Grade 8 and Grade 9 are required to have a new Health Inventory Form on file in the nurse’s office.

Additionally, for the school year 2016/2017, the Maryland Department of Health requires that all students in SEL and Grade 1 have at least two doses of Varicella Vaccine (unless they have had an active outbreak) and all students entering 7th and 8th and 9th grade have a single dose of Tdap and a single dose of Meningococcal (MCV4). These Health Forms along with the current vaccine requirements can be found here on the school website under ADMISSIONS > Forms in the “Health Forms” tab towards the bottom of the page.

These requirements are in addition to the existing school vaccination requirements listed in this OVERVIEW OF REQUIRED VACCINATIONS FOR THE SCHOOL YEAR 2016/17. If you choose not to follow the State of Maryland’s Vaccination Requirements due to religious reasons a Religious Exemption Form must be filled out and turned into the nurse’s office. This form can also be found here on our school website under ADMISSIONS > Forms in the “Health Forms” tab towards the bottom of the page.

You should work with your child’s doctor to get your child’s vaccination record or the vaccinations that are missing.  If you are unable to get an appointment with your child’s doctor, call your local Health Department at 240-777-0311.  The Health Department will be able to direct you to when and where vaccination clinics will be held over the summer months.

You must provide proof of required vaccinations or proof of a doctor’s appointment on the first day of school.  The appointment should be with 20 days of the first day of school.  The Health Department will visit our school to make sure our children are complaint with State Vaccine Requirements.  The Health Department has the right to begin proceedings to have your child exempt from attending school if the proper vaccines are not received.  In addition to the required vaccines, please talk to your child’s doctor about other vaccines such as the Flu, HPV and Hepatitis A that your child may need.
